Abstract

Ovarian cancer has a high mortality rate due to late diagnosis, justified by the absence of initial symptoms. Mutations in the BRCA1 gene increase the risk of the disease. The occurrence of ovarian cancer during pregnancy is rare and presents major treatment challenges. This work aims to report a case of ovarian cancer diagnosed during pregnancy, highlighting the diagnostic and therapeutic challenges involved. This is a 36-year-old pregnant woman, asymptomatic, with bilateral ovarian lesions detected in the 7th week of pregnancy on obstetric ultrasound. BRCA1 mutation and elevated CA125 were confirmed. After left oophorectomy, pathology diagnosed high-grade serous cancer. One month after surgery, in the 12th gestational week, a new lesion was identified on the right ovary by ultrasound. The patient underwent videolaparoscopy surgery and neoadjuvant chemotherapy. The cesarean section was scheduled for the 33rd week of pregnancy. This case report aims to demonstrate that early diagnosis is essential for a favorable outcome in cases of cancer during pregnancy and that a multidisciplinary approach is necessary to optimize maternal and fetal outcomes.
